Freckles are small, flat, light brown spots on the skin, darker than the surrounding area. They appear more frequently with continuous sun exposure, especially in fair-skinned individuals. Common areas include the cheekbones, nose, face, shoulders, and upper arms.1,2,3

Hori's nevus, also known as acquired bilateral nevus of Ota-like macules, is a skin condition characterized by multiple blue-grey to grey-brown spots on both cheeks. It typically appears in adulthood and is most common in middle-aged Asian women.4

Solar lentigines, also called sun spots or age spots, are flat brown or black patches caused by cumulative sun exposure. They commonly appear on the face, hands, and shoulders, especially in older adults.5,6,7

Pigment laser treatments can fade freckles by reducing melanin, the main cause of pigmentation. A combination of laser techniques for pigment reduction and removal typically requires approximately 3 treatments for noticeable pigmentation fading.
Hori’s nevus can be effectively treated with laser, which penetrates the skin to break pigments into tiny granules. White blood cells then remove these granules, reducing dark spots and preventing further pigmentation. The surrounding skin remains unaffected, while the treatment leaves it smoother and more radiant. Typically, about 6 sessions are needed to fade pigmentation.
Solar lentigo (Sun spots) can be effectively faded with laser treatment. Pigmentation typically lightens after 1–3 sessions.
There are 4 stages that involve the prevention of new melanin production:
Minimal downtime is required, and patients can resume daily activities immediately. A slight pinkish tint may appear and fade within 24 hours. Patients should avoid scrubbing but may wash their face as usual. Avoiding sun exposure for 5–7 days is recommended. If sun exposure is unavoidable, sunscreen must be applied for protection.

A patient presented to BSL Clinic with both freckles and solar lentigo skin conditions for treatment. She became uncomfortable in social situations and had anxiety in meeting new people. In the past she heavily used make-up foundations to conceal her skin conditions, but this was not a long-term solution.
